What is a Frenectomy?

A frenectomy is a simple surgical procedure performed to remove or loosen a band of tissue that connects the lips, cheeks, or tongue to the jawbone. This tissue is known as a frenum (plural: frenula). In some children, the frenula can be excessively tight or thick, leading to a range of issues.

  • Lip Frenulum: The lip frenulum is the tissue connecting the upper lip to the gums. An overly tight lip frenulum can cause issues with breastfeeding latching, speech, cleansibility of upper front teeth or even a gap between the front teeth.
  • Tongue Frenulum (Tongue Tie): An overly tight tongue frenulum can restrict tongue movement, leading to difficulties with breastfeeding latching, speech, swallowing and airway development.

When Does Your Child Need a Frenectomy?

The need for a frenectomy largely depends on the severity of the symptoms and the impact on your child's quality of life. Here are some signs that your child might need a frenectomy:

  • Breastfeeding Difficulties: If your newborn is having trouble latching or seems to be struggling with breastfeeding, it could be due to an overly tight tongue or lip frenulum.
  • Speech Issues: If your child is having trouble pronouncing certain sounds or words, a tongue tie could be the culprit.
  • Dental Problems: An overly tight lip frenulum can contribute to cavities of the front teeth because of restricted toothbrush access. It can also contribute to a gap between the front teeth and other tooth alignment issues.

How is a Frenectomy Performed?

A frenectomy is a quick procedure. It can be performed using multiple tools such as laser, electrocautery, or scalpel, and the choice largely depends on the doctor’s preference and the specifics of your child's case.

  • Scalpel Frenectomy: This traditional method involves using a scalpel to cut the frenulum. While effective, it can lead to more post-operative discomfort, bleeding and longer healing times.
  • Electrocautery Frenectomy: This modern method involves using an electric wire to cut and cauterize the frenulum simultaneously. It typically results in less post-operative discomfort and quicker healing times.
  • Laser Frenectomy: This newest method involves using a laser to cut and cauterize the frenulum simultaneously. It typically results in even less post-operative discomfort and quicker healing times. There are many types of lasers, some better than others and some no better than other methods.

Aftercare and Recovery

After the procedure, your child might experience some mild discomfort. However, most children recover quickly and can return to their normal routines within the week. Here are some aftercare tips:

  • Encourage your child to drink plenty of fluids to help soothe the area and prevent dehydration.
  • Soft foods may be more comfortable for your child to eat in the first few days following the procedure.
  • Over-the-counter pain relievers can help manage any discomfort, but always follow the recommended dosages.
  • Stretches. Your doctor will go over with you different considerations or techniques to help reduce the chance of the tie reattaching after treatment.
