If the privacy practices change, I will be offered a copy of the revised Statement of Privacy Practices at the time of my first visit after the revisions become effective. I may also obtain a revised Statement of Privacy Practices by requesting that one be emailed to me. This consent will remain in effect until terminated by me in writing.
I acknowledge that I have read the Statement of Privacy Practices in Accordance with the Health Insurance Portability & Accountability Act of 1996 (HIPAA), and am free to obtain a copy of it.
I authorize Campfire Smiles Pediatric Dentistry to release, obtain and share my child’s dental and health records with other dental and health care providers that are participating in my child’s care and treatment.
